Job Description :
This RN Case Manager position is in the Boston area and north to North Shore and in other areas north of the city covering multiple towns. The Agency is a well-established home health and hospice agency. They are nonprofit and mission driven. The agency offers a generous benefit package, limited weekend rotations for home care and none for hospice.
This is a Monday-Friday 37.50 hour, day position with some start time flexibility.
Required Qualifications :
Requirements include
Licensed RN in MA
1+ years of acute experience i hospital, SNF, Rehab etc
1+ years in Home care as an RN or RN hospice care visiting patients for care is preferred
