INTAKE RN OR LPN 32 HOURS

We are working with a client to find an Intake RN or LPN. This is a 32 hour position Saturday, Sunday, Monday and Tuesday (days). May consider weekends only.

Responsibilities include:

• Receiving and processing all referrals for home care


• Overseeing and Assess all referrals so that all processes are completed in a timely, efficient , professional and compassionate manner.

• Clinical assessment skills are required

• Exceptional communication skills


• Ability to be a team player and work effectively and compassionately with patients, families, co-workers, physicians, referral sources, management and support staff.


• Optimize use of resources for patients and families

• Work with a team of scheduler


• Work as a team with clinical managers


• Ability to be flexible and work effectively under pressure


• Ability to work independently with minimum supervision


• Excellent organizational and time management skills


• Caring and compassionate nature


• Computer skills - EMR and Microsoft Suite


• Ability to troubleshoot and problem solve

Education/Qualifications:

• Current Massachusetts License in good standing


• RN - or LPN required.


• 3+ years of nursing


• 1+ year in direct home health care


• 1+ year of triage experience


• Medicare and Managed Care experience required


• Knowledge of EMR
