RN Case Manager Home Health Care - Full time and Part time positions available!

New openings for Registered Nurses with recent home health care experience for this growing and well established, not-for profit home health care agency in CT!   The RN Case Manager will be a part of a multi-disciplinary team providing the highest quality home health care services to individuals in their homes throughout the community.  The RN Case Manager will educate patients and their families and work with them to develop and implement a patient care plan to provide the upmost quality care.

• RN w/ current CT License


• Recent experience in a Medicare Certified Home Health Care agency preferred


• Strong working knowledge of OASIS


• Excellent clinical and assessment skills


• Ability to communicate effectively with patients and families as well as referral sources,           physicians and other community resources


• McKesson/Netsmart experience a plus!    


• Excellent written and oral communication skills
